Bravo Wellness Portal
Welcome to your first step towards a healthier lifestyle! Follow these simple instructions to set up your Wellness Portal, where you can access tailored health tools and resources designed just for you.
Download the free MedMutual App from the Apple Store or Google Play.
Step 1: Open the MedMutual App and log in as a member.
Step 2: From the landing screen, select “View Dashboard”.
Step 3: From the Dashboard select “Launch Wellness Portal”. This will launch the Bravo wellness portal.
Step 1: Navigate to medmutual.com and log in as a member.
Step 2: From the Healthy Living tab in the main navigation, select “Wellness portal”.
Step 3: Accept the Terms. This will launch the Bravo wellness portal where you can follow the steps below.
Step 1: Select the Health Assessment Card.
Step 2: Read the additional information displayed on your screen. Select “Get Started →”.
Step 3: You will be directed to a new webpage to take the assessment. Click “Continue” and answer all questions.
Step 1: Select the Biometric Screening Card.
Step 2: Read the additional information displayed on your screen. Select which screening option you prefer (ONLY ONE IS REQUIRED).
Step 3: You will be directed to a new webpage for that particular option. Follow the instructions on the page.
Step 1: Select over the Preventive Care Service Card.
Step 2: Read the additional information displayed on your screen. Select “Download Preventive Care Submission Form →”.
Step 3: Download and print the form, take it to your appointment and have your provider sign it.
Step 4: Upload your completed, signed form to the portal.
Step 1: Select the three black circles on the lower right corner of the app window.
Step 2: A menu will popup on your screen. Select “Forms”
Step 3: Select the camera icon to upload your form.
Step 4: Choose to either take a new photo, or upload an existing photo or document from your phone. NOTE: If you choose “Take a photo”, you may need to update settings on your phone to give the app access to your camera.
Step 5: Once the photo or document is uploaded select “Use”, on the next screen that appears select “Submit” and you will get a message confirming the document has been submitted.
Step 1: Navigate to the Upload Forms section of the main navigation.
Step 2: On the Upload Forms page select “Select a file” or drag and drop the form into the blue dotted box on the page.
Step 3: Once you have uploaded the file select “Submit” and you will get a message confirming the document has been submitted.
Earn extra points by participating in wellness challenges or taking classes to learn how to improve your health. Scroll through the options available. Once you find one that interests you, click get started and follow the prompts on screen.
There are a host of additional resources available to you through your Medical Mutual Wellness Program. Navigate to the Resources page of the top navigation. Scroll through and once you find one that interests you, click through to learn more.
Complete the following tasks to earn the corresponding electronic gift code. Earned incentives are sent via email from wellness@workspring.org. Please allow for completion to show in your portal and each gift code to be sent in the timeframes listed:
Gift codes earned will be sent to the email registered in your WorkSpring Wellness portal.
You can view your rewards status at any time by clicking on the Rewards Summary tab at the top of the page.
